If you want to spice things up, just a taste, if you play them on PC REmake randomizer is really fun. It randomizes all the items there is also an enemy randomizer but you risk having the Tyrant spawn in rooms (I think you can disable it so he only spawns at the end though) still enjoyable.

Brain parasites you put into your own head, entrails of a crashed living ship as part of the map, that kind of stuff. There was a gross part in Dragon Age, really shocking and repulsive. But it was just one single part. This repulsiveness seems to be the fundament of BG3... I didn't see any of the sex stuff. But I hated the constant romancing mechanics intruding on you. I wanted to play it asexual. Fucking in games is boring. But all my party members tried to romance me. It bothered me that the game forced me to reject them. At least it bothered me when I started. Now I hate the game and let the NPCs have it by being a prick. Whenever someone cries to me about violent loss, I call them cowards for not saving their loved ones.

There is also a door randomizer which is as crazy as it sounds. I'd recommend starting with the item one or just go all fawkin in and randomize everything. Seeing sharks in other rooms is pretty funny.
How difficult is it compared to FromSoftware games?
About the same, areas before bosses are a bit easier I'd say but the boss fights are on par with FromSoft difficulty. There is a really cool weapon system where you can swap out every blade and handle and mix and match them, for example you can put a dagger blade on a polearm handle and vice versa.

This is a major problem for all modern 'rpgs'. Where if your character interacts with a potential romance partner in any way then the next thing you know they will be trying to fuck you or actually fucking you. When the incel mental cases write the dialog and romance options you won't be getting anything normal of course. But the games clearly are trying to force your hand with the sex options by constantly putting them front and center. "Hey did you just kill that dragon? Let's suck each other's dicks and orgy near its carcass!". I'm just trying to play a knights versus dragons game for fuck's sake.
There are compilations of the bestiality and demonic romance options in BG3. They are endless.
