He basically details how HIV and sex per se don't cause AIDS, but literal filth and total malnutrition do because they're the direct cause of the breakdown of the immune system. Shitloads of people have specific immunodeficiency viruses and disorders, but they aren't sexually transmitted and they don't directly lead to death.
