The obvious answer is that he's utterly insane, which he is, but I also think he copes by telling himself that "fat" only applies to the 400 lb morbidly obese in motorised scooters with attached oxygen tank types he probably sees littered around Wisconsin. Pat is the worst kind of fat. He really isn't a fatass. You can see in his younger, old testament days he was a skinny weak faggot.

A terrible diet, slobbish lifestyle, and alcohol addiction have bloated his torso, face, and breasts. His legs and arms have remained the same. It's like 4 skinny limp poles holding up a tub of goo.
