What happens to a narcissist in the long run?

Their actions catch up with them. All the partners they abused and cheated on want nothing to do with them, and won’t even speak to them.

The “friends” they made along the way likewise, fed up of being ignored and sidelined and made to feel unimportant.

Even their kids are usually estranged- even if they know nothing about narcissism, they know selfishness and coldness, and have moved on with their lives, to the exclusion of the narc.

Old age for a narc can be a very lonely place, as all their past misdeeds come home to roost. If they are very lucky, they have hung on to a supply, who is at least there to provide home comforts and a clean house, but that supply will be hostile and prickly, sick of being badly treated, and now might be the time for some revenge, so the atmosphere at home is generally toxic.

You will never see a happy, contented old narc, surrounded by a loving family and great friends- it is likely to be the very opposite.

Karma? I believe so, they deserve nothing better.

If he wizens up, by his 50s he can be where he should have been in his mid 20s, and get a real 1.7 highschool GPA-esque career. Like an assistant to an Auto insurance claims adjuster or something.
