I've read some articles about this and the most likely cause is contamination when they cut and bag coke in the same location as the cut and bag fentanyl. There is also the possibility that it's a pre-mixed speedball package for IV drug users so inclined and they occasionally fall into the hands of those just looking for coke. From what I've read, nearly all instances of fentanyl-laced coke involved real ghetto coke, total street level shit. They don't exactly have really high quality control standards in that world.
