I used to think that he spent like an hour every morning letting out pained, primal screams while he stares at himself in the mirror, squeezing his big fat tits as hard as he can like he wants to rip them off. I don't think that anymore. He's a delusional, crazy fuck.

I think he's one of these people who ALWAYS lie about their talents and achievements. So much so that they believe their own BS.
Do people like this ever have a moment of realisation? Or do they continue to blame everything on other people, refusing to accept that they are the architect of their own destruction?

I'd imagine that Patrick is self medicating with alcohol. Every time he has a negative thought (ie reality creeps in) he quickly reaches for the bottle. This kind of emotional drinking is definitely the start of a serious abuse problem.
It's possible he has borderline personality disorder.
But there is some good news!
BPD can be a serious condition, and many people with the condition self-harm and attempt suicide.
