His doctors might be afraid that his ol ticker HA HA HA HOLEE SHIT can't handle any level of withdrawal so they went with the one alcoholic beverage that has putative benefits for heart health. They don't want to encourage the old lush so they set an arbitrary restriction in the hopes that he doesn't immediately drink himself to death.

Doctors have been known to not only recommend but even prescribe alcohol to alcoholics. If they think you're more likely to face imminent death from withdrawal due to health issues (recent quadruple bypass for instance) they'll tell you to have x drinks per day.
People abuse their prescriptions all the time but sometimes it's safer to let them do it than to stop them.
