I used to have one of those pick-punch things and I gave it to a friend who ended up giving it to a friend, and so on.
As has been mentioned, they don't cut clean, there's little burrs on the edges and the edges are half assed sharp. I never bothered sanding them down with some fine grit sandpaper because that's a retarded bit of effort for a free guitar pick.

Honestly it's better just to cut something out roughly with a sharp knife. I have a card or two in my wallet that I'll whittle down if I lose all the picks in my gig bag.
